It is extremely important to answer all questions truthfully and to the best of your knowledge. All donors are legally obliged to disclose accurate information and to inform the clinic of any additional relevant medical information, if not specified within the application form. Please complete the application as comprehensively as possible, as incomplete applications will not be accepted.
